Service
Coordination is the process of helping
families and children receive all the
community and Help Me Grow services they
choose for their child and family. The
role of the coordinator is to collaborate
with the family to reach their individual
needs.
A
service coordinator can provide assistance
and support to families as the family
builds on their strengths and resources.
How does the process work?
Based
upon a family's concerns, priorities, and
resources, a service coordinator helps the
family find and use community services which
will benefit their child and family.
Home Visiting
Service
Coordinators also provide home visiting
services. These meetings are scheduled
according to the family's needs and routines.
A Family Support Specialist (FSS) is also
available to meet the ever-changing needs
of our families. The FSS provides Help
Me Grow families with linkage to other
parents in the community, support groups,
customized information pertaining to each
family, and can be the support the family
utilizes to navigate the agencies and
programs within Hocking County.
