The
Individualized Family Service Plan (IFSP)
is the foundation of services that are
family-centered. It is based on your child's
strengths and your family's concerns and
priorities.
The
IFSP is a way to share concerns, information,
and needs with your service coordinator
and other selected individuals. The process
includes sharing information the family
feels is important, planning times, transportation,
and people to meet with, stating what
goals the family would like to achieve,
and outlining who is responsible for the
steps taken to complete these goals.

The
Hocking County Board of MR/DD serves children
birth to age three who are experiencing
one or more delays in their development.
The developmental Specialist works with
children though a center-based and home-based
program to facilitate and stimulate development.
In
addition, MR/DD provides speech, occupational,
and physical therapies at the family Resource
Center.
